Milwaukee Health Care Partnership (MHCP) is a private/public consortium dedicated to improving health outcomes, advancing health equity, and lowering the total cost of care. MHCPs membership includes health systems, community health centers, and city, county, and state government agencies.
Every three years, the MHCP’s health system members – Aurora Health Care, Ascension Wisconsin, Children’s Wisconsin, and Froedtert Health — share the funding and design of a Milwaukee County Community Health Needs Assessment (CHNA) to facilitate shared learning, the alignment of strategy, and reduced duplication of resources. The health system’s latest shared CHNA serves as a leading example of an authentic community collaborative process for health improvement. Milwaukee committed to identifying and addressing the root causes of health inequities, including racism and discrimination, in the CHNA process and subsequent strategy development.
