To address their lack of data and tools for population health planning, the Orange County Health Improvement Partnership (OCHIP) determined the need for a central, online platform that would support multi-stakeholder collaboration, increase understanding and alignment of community needs, and support Public Health Accreditation Board (PHAB) accreditation. OCHIP utilized the HCI Platform to fulfill this need and seek further funding to continue their community health improvement work.
Orange County was awarded a CDC: Partnerships to Improve Community Health grant as a result of efficient community collaboration and utilization of robust data, supporting their ability to focus effectively on underserved populations.
